Sing Sing Program: Help! We are going to do a program at Sing Sing in NY for inmates as a re-entry program giving self-advocacy, communications and mental health skills and strategies among other things and need help talking through identifying both sponsorships and grants.

The Healthcare Navigation Project is an independent 501(c)(3) intentionally designed to disrupt our complex healthcare system with a health literacy and mental health life-skills program for ALL youth transitioning to adult care (HCT), and everyone else. This includes rare or chronic disease, underserved communities, or those with educational disabilities. We bridge the gap in healthcare by empowering all people to learn to actually use healthcare and self-advocate before they need it instead of after when it's too late. We empower people.
